INTRODUCTION The presence of ANA is associated with systemic rheumatic diseases. Indirect immunofluorescence (IIF) on HEp2 cells is the standard technique for the detection of ANA (1). Detection of ANA reactivity by IIF is a sensitive technique, but as the specificity is low, additional tests are required for confirmation and more precise identification of these antibodies. Therefore tests for antibodies to double-stranded DNA (dsDNA) and to extractable nuclear antigen (ENA) are subsequently performed in patients with a positive ANA result. These tests are an important part of the diagnostic workup of systemic rheumatic diseases and they can assist the clinician in discriminating between the different types of systemic rheumatic diseases. For instance, anti-dsDNA and anti-Sm antibodies are indicative for systemic lupus erythematosus (2, 3). Moreover anti-dsDNA antibodies can help identify a subgroup with high risk of lupus nephritis (4, 5). Besides their diagnostic value, some antigens also have prognostic importance, e.g. serum levels of anti-Jo-1 antibodies strongly correlate with disease activity (6). The probability of detecting ANA and anti-ENA antibodies can be useful in developing a testing strategy for the detection of autoantibodies. However few data have been published. Peene (7), Kang (8), and Dellavance (9), have analysed data from cohorts of serum samples in academic hospital labs. No such data are available for a large community hospital. The aim of the current analysis was (a) to determine the prevalence of ANA positivity in a large and consecutive cohort of serum samples referred for ANA testing in a large community hospital, (b) to provide data on the probability of detecting more specific antinuclear reactivities (anti-ENA and anti-dsDNA) in these samples after a positive ANA screening test and (c) to identify variables predicting positivity for anti-ENA and anti-dsDNA antibodies.
